If you are under 19 years of age you must complete the MINOR RELEASE which can be found here

NOTE: For it to be valid, you must print it out in duplicate and in color. It must be signed by BOTH parents or all guardians and be notarized.

In the date part at the top, if we put "all events" it will cover all events for that year.
